A plugged exhaust is an issue for your engine.

Above is an example of a new exhaust system that is not plugged.
When your exhaust is plugged on any engine it can cause excess back pressure. This back pressure does not allow air to flow properly. It traps exhaust gases instead of letting them flow out properly. This means that your engine has to work harder than it should to do it's job.
When this happens it can have several issues that are hidden due to the plugged exhaust. Typically what we see is that power drops in your engine because the engine is not breathing properly. Then your fuel economy will suffer, this is due to your engine needing to work harder to produce power and it takes more effort to run. The exhaust heat can build up, this an and may damage the turbocharger, the exhaust valves, manifold, or other emissions components. When the exhaust is completely plugged it can cause the engine to run poorly, cause smoking, go into limp mode and possibly stall.
On a diesel, a restriction can also interfere with turbo performance. The turbocharger depends on smooth exhaust flow, so a blockage can reduce boost and make the truck feel slow to respond. If the restriction is tied to the DPF or another emissions part, it can also trigger warning lights and regeneration problems. This can also mask other issues.
